What happens if I miss an appointment?

Missed appointments or appointments that are canceled less than 24 hours prior will result in a $100.00 charge. All fees must be paid before appointments are able to reschedule or prescriptions are able to be refilled. Charges may be applied on the same day as the missed appointment.

Are appointments available virtually?

Yes, I offer both in-person and virtual appointments. If your appointment is virtual you will receive a link via text or email from simple practice. You can always call the office if you are having issues logging on or send a message via simple practice app.

How do I contact my provider?

For the fastest response, we recommend using the online patient portal (simple practice) to send your provider a message. For patients calling the office directly, please allow one to two business days for a response. Please note that leaving multiple messages in multiple areas will not make for a faster response.

What if I need to contact my provider after hours?

After-hours care is not provided at ELE-Mental Wellness. For emergencies outside normal business hours, please seek care at your local emergency room, call 988, or the 24-hour mobile crisis hotline at 844.709.4097.

How do I request a refill?

Refill requests may also be made through the online patient portal (simple practice).
